| U.S. Drug Enforcement Administration | Practitioner shall prescribe Schedule II through V controlled substances only for legitimate medical purposes and within the scope of his professional practice. At no time shall practitioner procure, store, administer, or dispense controlled substances under his DEA registration to include samples. Shall not prescribe/distribute controlled substances to himself, family members, fellow employees, or any of their immediate families. Shall not consume any controlled substances that are not prescribed or dispensed to him or another licensed physician acting in the usual course of professional practice to treat a legitimate medical condition. Shall maintain electronic and clearly legible logs of all of his controlled substances prescriptions. |
