I practice medicine in a transition clinic where we see patients who have just been discharged from the hospital. Our primary goals are to help prevent readmissions and help connect patients with resources in the community to help them be successful with remaining healthy through management of acute and chronic conditions. We especially work with persons who are uninsured or do not have a primary care physician/provider.
